The Public Health Nursing/Family Case Management Program offers home visits by a registered nurse or case manager to low income families with pregnant women, infants and high risk children up to age 2.

Sexually Transmitted Infections/HIV Prevention


The purpose of the STI/HIV program is to provide comprehensive care and prevention services for at-risk individuals in Lake County. The program also also provides health education, treatment adherence counseling, psycho-social support and linkage to care and partner services to individuals living with HIV.

Women, Infants, and Children (WIC)
