COURT FORMS

There are many forms available for various case types.  Those case types include: Adult Name Change, Civil/Small Claims, Criminal/Traffic, Divorce/Family, Minor Name Change and Probate.  Other than providing these forms, the Circuit Clerk’s Office is prohibited from giving any legal advice.

All of these forms were designed to be “fillable”, so that the user can fill in the blanks using their computer.  Completed forms can then be printed and filed in court or with the Clerk of the Circuit Court.
