The ServicePoint® project envisions every healthcare and human service consumer in Lake County having access to the complete array of effective and comprehensive services and care which will lead to improved health outcomes. Human service consumers continue to present with more complex and layered needs. The ability of the providers to collaborate and coordinate their care will enhance the quality of life of their clients. Often, that care is provided by multiple organizations. ServicePoint®, an integrated technology platform, is one key to answering this question and identifying gaps in the services provided.
The goals of the ServicePoint® project are:
1. Increasing collaboration among Lake County healthcare and human services agencies
2. Reducing barriers to service for clients accessing services at multiple agencies
3. Measuring system performance and collective impact across multiple agencies
