FY 2013 – FY 2014 Continuum of Care (CoC) Application
Links to all of the materials necessary to complete an agency’s portion of the application for Continuum of Care funding can be found below. All agencies who currently receive funding through the CoC need to submit an application and Exhibit 2. All agencies interested in new funding for permanent supportive housing for the chronically homeless or rapid rehousing for families, needs to respond to the Notice of Intent (NOI).
Please note that agencies need to submit an application for projects funded through HUD’s Continuum of Care by 5 pm on Friday December 20th 2013. The completed applications must be e-mailed to Melissa Albach at email@example.com.
Lake County has grant funds available from the US Department of Housing and Urban Development to assist low-income persons. Funds can be used for the following activities:

Community Development activities are overseen by the Community Development Commission (CDC), which meets the fourth Wednesday of every month at 7:00 PM.
The CDC encourages residents to get involved in the decision making process for the use of grant funds. The Citizen Participation & Input page provides information on how you can do so.
