Lake County has developed this Request a County Service program in order to provide residents an opportunity to submit requests for service from their county government. Residents may use the online form below to complete an interactive service request form. The form will be sent to the appropriate County department for them to take action.

Request a County Service will provide:

• Improved customer service
• Interactive submittal and recording of requests to ensure timely response and follow-up
• Enhanced system for tracking requests for service 

Currently, Request a County Service provides residents an opportunity to report or request help with such specific items as:

• Damaged mailbox from snowplow
• Dead animal in roadway
• Dead animals
• Nuisance reports
• Potholes
• Problems with animals
• Restaurant complaints
• Septic issues
• Signs missing from roadways
• Surface water issues

The Request a County Service program will be expanded in the future depending on the frequency of requests for specific services. If you would like to see a service added to this program, please email communications@lakecountyil.gov.
